Practice Field Reservation Request
Please review the terms and conditions and provide the required information to submit a reservation request for using the practice field at Eastview Community Center. Only approved reservation requests will ensure use of the practice field for a specified date and time.
TERMS AND CONDITIONS
By submitting this reservation request, the individual named below agrees to the following terms and conditions:
The individual named below is at least 18 years of age and accepts full responsibility for the actions of team's players, parents, coaches and team's guests
Prior approval from Eastview Community Center is required for erecting tents, shelters or inflatables on the grounds; using amplified sound; blocking the drive and parking; or driving vehicles in the grass
To provide a family-friendly and safe environment, no alcohol or tobacco products are permitted on the property
A cellular phone must be on hand in case of emergency
Eastview Community Center may schedule activities in the facility or on the grounds during use of the practice field
Eastview Community Center reserves the right to reschedule a field reservation for community-focused events; Every attempt will be made to give as much advance notice as possible if this occurs
Failure to cancel unneeded reservation(s) or abide to these terms and conditions may result in loss of future reservations and use of the practice field
